Discharge Planner Assistant

Position Description:
This individual supports the case management care team coordinating safe and timely patient transitions from the acute care setting to the appropriate next site of care.
Duties:
• Assists the case manager and health care team with the communication and collaboration of the discharge care plan including initiating referrals, i.e. home health, DME, arrange transportation, providing patient choice forms, and making follow up medical appointments
• Data entry of case related information, submit referrals to facilities and follows-up on bed status and possible bed offers, submit DME necessary referral paperwork and coordinates delivery of equipment upon request of the Case Manager. Provides clerical support to department as needed.
• Delivers Important Message from Medicare to all Medicare patients prior to discharge. (4 to 48 hours prior to discharge) according to CMS guidelines.
• Assist and work closely with Case Managers, Social Workers, physicians, nurse managers, nursing supervisors, and other members of the health care team to achieve desired outcomes for his/her caseload such as researching patient information, informing patients of their right to choose when referral to SNF, Home Health Care (HHC), etc. and other duties as requested.
• Other duties and responsibilities as assigned.

Qualifications
Requirements:
• High School diploma or GED.
• 3 years’ experience in patient care or other related healthcare preferred. Previous insurance knowledge or clinical work experience in “front or back” office. Strong interpersonal, communication, organizational, and problem-solving skills required. Comprehensive computer skills required. Ability to work independently and meet deadlines. Possess strong critical thinking skills.
• Current CNA certification preferred.
