Position Overview
Our Endocrinology Practice is seeking a dedicated and detail-oriented Diabetes Program Coordinator to lead the development, implementation, and ongoing management of our diabetes care and education programs. This role is ideal for a compassionate healthcare professional who is passionate about improving patient outcomes through education, care coordination, and community engagement.
Key Responsibilities
• Coordinate and oversee day-to-day operations of the diabetes management program
• Develop, implement, and evaluate diabetes education materials and workshops for patients
• Collaborate with physicians, nurses, dietitians, and other care team members to ensure comprehensive, patient-centered care
• Monitor patient progress and outcomes, adjusting care plans and interventions as needed
• Maintain accurate patient records and documentation in compliance with HIPAA and organizational policies
• Serve as a resource and point of contact for patients managing diabetes, providing education on blood glucose monitoring, medication management, nutrition, and lifestyle modifications
• Track program metrics and prepare reports on patient outcomes and program effectiveness
• Stay current on diabetes care guidelines, best practices, and emerging research
• Assist with grant reporting, community outreach, and partnership development as applicable
• Support certification or accreditation requirements for the diabetes program (e.g., ADA or AADE recognition, if applicable)

Qualifications
• Bachelor's Degree in Nursing. Current license within the Commonwealth of Massachusetts as a Registered Nurse; CDE Certification.
• Minimum of 1–3 years of clinical nursing experience, including at least 3 years of supervisory experience.
• Experience as a diabetes educator in an ambulatory, community health setting.
• Knowledge of all aspects of diabetes care and the ability to act as a resource to patient, community and hospital personnel.
