Shift: Days 8:00am - 5:00pm
Status: Full time
Compensation: $45,000K — $55,000K yearly
The estimated range is the budgeted amount for this position. Final offers are based on various factors, including skill set, experience, location, qualifications and comparisons to colleagues in similar roles.

Position Purpose: Provides services to clients based on client needs, areas of professional knowledge and skill, and in accordance with the mission and scope of services of the Community Wellness Program. Helps clients access health care, social services, and public benefits. Facilitates community-based education and support workshops, classes and groups. Participates in community health and wellness activities. Assure that Community Wellness complies with applicable grants, service contracts and reporting systems.
Job Summary:
• Works with self-pay patients to determine Medicaid or Medicare eligibility, collects required documentation and submits public insurance applications to HFS, monitors application approval status, files appeals for denials, selects appropriate MCO plan and provider for patients, enters data for all activity in the hospital EMR system (Meditech).
• Engages with clients, assesses basic needs and provides basic and targeted case management services, including facilitating appropriate medical and social service referrals. Assists clients in the application process for various public benefit programs, including Medicare, Medicaid, and SNAP.
• Participates in program planning and evaluation activities. Is also accountable for monitoring, documenting, and tracking program and client data and assuring compliance with grant-related or hospital-related documentation requests made by the supervisor.
• Represents the Community Wellness Program at public forums and meetings and builds relationships with organizations and individuals with whom the Community Wellness Program has an invested interest or partnership, including other departments of Saint Anthony Hospital, Health Affiliates, physicians, schools, churches, and community-based social service agencies.
• Communicates with and maintains relationships with local, regional and state benefit program offices and other organizations with which Saint Anthony Hospital and the Community Wellness Program have working relationships.
• Participates in, plans and executes community-based public benefits education and outreach activities, including health fairs, flyer distribution, and presentations.
• Actively contributes to and participates in supervision, staff meetings, and in-service.
Marginal Functions
• Performs both essential and marginal job functions in a safe manner as identified by Saint Anthony Hospital and the respective department while adhering to the Hospital’s confidentiality norms and standards. Actively and consistently demonstrates the core values of Saint Anthony Hospital in all interactions with others.
• Assumes responsibility for knowing and maintaining established hospital and departmental objectives and policies/procedures, including general sanitation, safety, environmental and infection control standards.
• Participates in performance activities including problem identification, data collection, solution selection, implementation and evaluation. Also assists in the care and maintenance of departmental equipment and supplies as appropriate.
• Responsible for sales, promotion and marketing of Saint Anthony Hospital. Develop and maintain strong business and referral relationships with physicians, affiliate clinics of Saint Anthony Hospital and community-based organizations.
Job Requirements
• Bachelor's Degree plus a minimum of one year work experience related to the job description
• Basic knowledge of public benefits
• Experience with data entry
• Familiarity with community resources in the hospital’s service area
• Comfort and familiarity with Latino and/or African American cultures
• Excellent interpersonal skills
• Bilingual English/Spanish
• Must be able to work some outreach events during evenings or weekends
