The Community Outreach Specialist, Senior Services serves as the front door to our senior services program, building awareness, trust, and enrollment by connecting with older adults where they live, worship, gather, and seek support. This role is ideal for someone who is passionate about community engagement, relationship-building, and improving the health and well-being of seniors through meaningful outreach and access to care.
Minimum Qualifications
• High school diploma or equivalent required; Associate's or Bachelor's degree in a health, human services, or related field preferred.
• Minimum two (2) years of community outreach, community health worker, promotora, patient navigation, member engagement, or comparable community-facing experience.
• Demonstrated ability to build trust and maintain relationships with community organizations and community leaders.
• Strong verbal communication and public speaking skills.
• Ability to work primarily in the field, including evenings and weekends as needed.
• Strong organizational skills and basic computer/data entry proficiency.

Preferred Qualifications
• Community Health Worker (CHW) certification.
• Existing relationships with community or faith-based organizations within the SJCH service area.
• Experience with enrollment, eligibility, benefits navigation, or patient engagement programs.
• Experience working in a Federally Qualified Health Center (FQHC), community clinic, or safety-net healthcare setting.
• Bilingual skills (English/Spanish) highly preferred.

Responsibilities
• Establish and maintain relationships with community and faith-based organizations, senior centers, ministries, and local groups serving older adults.
• Represent St. John's Community Health at community events, health fairs, outreach activities, and partner meetings.
• Recruit and engage senior patients who may benefit from healthcare and supportive services.
• Conduct individual and group presentations regarding program services, eligibility requirements, and participation opportunities.
• Guide seniors and family members through intake, enrollment, and service access processes.
• Follow up with prospective participants and assist in overcoming barriers such as transportation, scheduling, language access, and documentation.
• Conduct reminder and re-engagement outreach for participants with declining participation or attendance.
• Connect seniors to appropriate SJCH programs and external community resources.
• Support the development and distribution of outreach materials tailored to the communities served.
• Maintain accurate records of outreach activities, referrals, presentations, and enrollment outcomes.
• Track and report outreach metrics, participation trends, and community feedback to leadership.
• Identify emerging community needs and advocate for service improvements that enhance access and engagement for seniors.

Skills
The successful candidate will possess exceptional interpersonal and relationship-building skills, allowing them to establish credibility and trust with seniors, families, community leaders, and partner organizations. Strong communication and presentation skills are essential for delivering outreach activities and educational sessions across diverse populations. The ability to work independently in a field-based environment, maintain organized records, and consistently follow through on outreach efforts is critical to success in this role. Candidates should demonstrate cultural humility, compassion, persistence, and a commitment to improving access to healthcare and community resources for older adults.
