Job Summary
The Community Health Outreach Case Manager is part of an interdisciplinary team that includes, but is not limited to, medical, case management and community development. Serves as patient advocate and assists in identification and improvement of service delivery. Ensure patients remain or become engaged in services and are educated about self-care issues. Develop community relationships with the support of the community health team assuring access to care barriers are identified and addressed.
Qualifications
Education
• BA/BS in sociology, psychology, social work, or related field

Experience
• Some prior community based outreach experience, previous experience providing health education
