Drawing on experience in social services, human services, community programs, advocacy, or a related field, the Navigator helps patients and families understand their options and take practical next steps toward accessing health care, social services, and community resources. The Navigator meets with patients to learn what support they are seeking, explain available options in plain language, and help them build confidence navigating systems. The role emphasizes personal connection, patient choice, and self-advocacy.
Essential Responsibilities
Patient-Facing Navigation and Support
• Meet with patients and families in person, when possible, and build trusting relationships through respectful, compassionate, and culturally responsive conversations.
• Listen to patients’ concerns and help them identify the support or information they need.
• Explain how to access health care, insurance, social services, public benefits, and community programs.
• Help patients understand referrals, prepare for appointments, use patient portals, and determine what steps to take next.
• Connect patients with appropriate community resources and explain how to contact or apply for services.
• Use plain-language communication, teach-back, and other supportive approaches to help patients understand their options.
• Encourage patients to make their own informed choices and strengthen their confidence in advocating for themselves.
• Help patients identify practical barriers—such as transportation, cost, language, or difficulty understanding a process—and explore possible next steps with them.
• Follow up with patients, as appropriate, to ask whether they were able to access a resource or whether they need help understanding what to do next.
• Maintain professional, supportive relationships while respecting patients’ independence, privacy, and personal decisions.

Patient Experience
• Help patients feel welcomed, heard, and respected when seeking assistance.
• Adapt communication to patients’ language, needs, and level of familiarity with health and social service systems.
• Share patient feedback with a supervisor or appropriate team when a recurring barrier or confusing process affects patients’ ability to get support.
• Partner with other staff as needed to help patients access appropriate assistance, while keeping the Navigator’s role focused on direct patient support.

Documentation
• Complete brief and timely documentation of patient interactions, resources discussed, and follow-up needs in accordance with organizational policies.
• Maintain confidentiality and follow applicable health center procedures.

Qualifications Required
• Experience supporting individuals, families, or communities through social services, human services, education, community programs, advocacy, or a related field.
• Ability to build trusting relationships with people from diverse backgrounds.
• Strong listening, communication, organization, and problem-solving skills.
• Compassionate, patient-centered approach and comfort working directly with people.
• Ability and willingness to learn health care and social service systems, relevant technology, and documentation practices.
• Ability to maintain confidentiality and follow organizational policies.

• Experience helping people access public benefits or community-based services.
• Healthcare, public health, behavioral health, or patient-navigation experience.
