About the Role:
The Coder II ER plays a critical role in accurately translating emergency room medical documentation into standardized codes used for billing, reporting, and analysis. This position ensures that all ER visit records are coded in compliance with current healthcare regulations and coding standards, directly impacting reimbursement and quality metrics. The role requires meticulous attention to detail to review clinical documentation, identify relevant diagnoses and procedures, and assign appropriate codes. Collaboration with healthcare providers and billing teams is essential to clarify documentation and resolve discrepancies.
Minimum Qualifications:
• High school diploma or equivalent; completion of a formal medical coding training program.
• Certified Coding Specialist (CCS) credential is required. Certified Outpatient Coder (COC), Registered Health Information Technician (RHIT), or Registered Health Information Administrator (RHI)
• Minimum of 2 years of experience in medical coding, preferably with a focus on emergency room or acute care settings.
• Strong knowledge of ICD-10-CM, CPT, and HCPCS coding systems and guidelines.
• Familiarity with electronic health record (EHR) systems and coding software.

Preferred Qualifications:
• Associate degree or higher in Health Information Management or related field.
• Experience working in a hospital or emergency department environment.
• Strong understanding of payer-specific coding and billing requirements.

Responsibilities:
• Review and analyze emergency room medical records to extract pertinent clinical information for coding.
• Assign accurate ICD-10-CM, CPT, and HCPCS codes to ER visits in accordance with official coding guidelines and payer requirements.
• Ensure compliance with federal, state, and organizational regulations related to medical coding and documentation.
• Collaborate with physicians, nurses, and other healthcare professionals to clarify documentation and ensure coding accuracy.
• Maintain up-to-date knowledge of coding standards, payer policies, and industry best practices through continuous education.
• Assist in preparing reports related to coding productivity, quality, and compliance as requested by management.

Skills:
The required skills enable the coder to accurately interpret complex clinical documentation and apply the correct codes, which is essential for proper billing and compliance. Attention to detail and analytical skills are used daily to ensure that all codes reflect the patient's diagnosis and treatment accurately. Communication skills are vital for interacting with healthcare providers to resolve documentation issues and clarify clinical information. Familiarity with coding software and electronic health records facilitates efficient and accurate data entry and retrieval. Preferred skills such as the coder's ability to maintain high-quality coding standards and support organizational compliance efforts.
