Job description

Purpose of Position:
Process and analyze medical records and charts for appropriate diagnostic and procedural codes for effective data retrieval, analysis, and claims processing at an entry level with limited experience.
Uphold the organization’s vision, mission, and corporate values. Demonstrate understanding of and compliance with organization’s policies, procedures, code of conduct and work rules.
Essential Functions:
• Assign proper codes to charts for outpatient encounters, using ICD-9-CM, to all diagnosis and procedures and CPT codes using coding guidelines and conventions
• Verify patient information from patient records and assure that all diagnosis and procedures are coded according to coding guidelines
• Provide regular statistical and analytical information for completion of reports
• Make recommendations for revisions to in-house forms and/or checklists for efficiency,compliance and coding changes
• Perform other duties as assigned
