Clinical Service Navigator
Location: Winston-Salem, North Carolina
Department: Neuro & Sleep Clinic
Employment Type: Part Time, 32 hours per week. Monday-Thursday 8am-5pm
Position Overview
• Assist clinic with rooming of patients.
• Triage patient calls.
• Explain all processes to patients in plain language and utilize teach back to ensure understanding.
• Provide exceptional patient care, demonstrating empathy and professionalism.
• Collaborate with a multidisciplinary team to deliver high-quality healthcare services.
• Be active participant in stroke support group.
• Monitor schedule for appropriateness.
• Call patients post discharge from hospital to ensure continuity of care.

What We're Looking For
• Education:
• High School or GED, required.
• 4 Year / Bachelors Degree, preferred.
• Experience :
• Three to five years of nursing experience, required.
• Two years of experience in care of targeted patient population; experience in a broad range of clinical situations or settings, preferred.
• Licensure/Certification :
• RN license in Appropriate State, REQUIRED
• Additional Skills/Requirements (required ):
• Ability to successfully complete generic and department-specific skills validation and competency testing.
