Job Requirements
This position will be supporting the Emergency Department at Holmes Regional Medical Center. The schedule will vary and will include 2nd shifts.
POSITION SUMMARY
The Clinical Pharmacy Specialist supports the planning, coordination, and daily execution of clinical pharmacy services, including active participation in operational workflows, such as medication order processing, clinical consultation and verification, and pharmacy monitoring to ensure safe and efficient patient care delivery. The Clinical Pharmacy Specialist facilitates the development, implementation, and maintenance of medication-use systems and evidence-based pharmacotherapy that promote optimal patient outcomes using safe and cost-effective drug therapies.
PRIMARY ACCOUNTABILITIES
• Assists in the implementation of Health First's mission and execution of the overall strategic goals as described by senior leadership, while participating in quality improvement activities both within pharmacy and interdepartmental where appropriate.
• Collaborates with pharmacy and interdisciplinary stakeholders to facilitate project management for developing, implementing and follow-up of medication education, guidelines, protocols, order sets and formulary changes as approved by appropriate committees.
• Performs drug class reviews, Medication Use Evaluations (MUEs), adverse drug reaction reviews, formulary reviews while contributing to continuous quality improvement, medication safety and financial stewardship.
• Monitors medication therapy regimens for contraindications, drug-drug interactions, drug-food interactions, adverse drug reactions and appropriateness.
• Collects and analyzes data, maintains documentation of clinical activities and prepares reports on pharmaceutical care and pharmacoeconomic activities related to financial impact and stewardship.
• Provides consultation, drug information and education to physicians, nurses and patients regarding pharmaceutical care and medication safety, suggesting evidence-based, cost-effective therapeutic alternatives to medical staff as needed
• Participates in the review of policies and procedures for pharmacy services and medication processes (e.g. therapeutic interchange) and non-formulary medication requests.
• Builds collaborative relationships, participates in quality and committee meetings, reviews key communications, and supports professional advancement through presentations, journal clubs and topic discussions.
• Reads and evaluates the medical literature for studies supporting appropriate pharmaceutical care and pharmacoeconomic programs, disseminating best practice information obtained from outside resources and across the Health First Integrated Delivery Network (IDN).
• Engages as a preceptor, educator and mentor for pharmacy students, residents and Health First associates actively through direct instruction and modeling to provide actionable feedback on the learner’s experience and performance.
• Reviews, verifies, and validates medication orders for accuracy, appropriateness, safety, and regulatory compliance.
• Oversees and supports medication-use system workflows, including medication preparation, dispensing, sterile compounding verification, inventory management, and timely medication distribution to ensure safe, efficient, and compliant pharmacy operations.

Work Experience
MINIMUM QUALIFICATIONS
• Education: Bachelor’s degree in Pharmacy (BPharm).
• Work Experience: Any one of the following:
• Three (3) years’ clinical pharmacy experience in pharmacotherapy decision-making, direct patient care and pharmacy operations OR
• Completion of an ASHP accredited residency.
• Licensure : Pharmacist licensure in the State of Florida.
• Certification : Any one of the following prior to start date and maintained:

• American Heart Association Basic Life Support (AHA BLS) Healthcare Provider Completion Card OR
• American Heart Association Advanced Cardiac Life Support (AHA ACLS) AND Pediatric Advanced Life Support (AHA PALS) Healthcare Provider Completion Card.
• Board of Pharmacy Specialist certification within one (1) year of start date and maintained.

• Skills/Knowledge/Abilities:
• Advanced knowledge of pharmacotherapy, disease state management and evidence-based medicine
• Participation in medication-use evaluations, formulary review & management and quality improvement methodologies
• Possess strong computer and technological skills
• Excellent interpersonal, organizational and problem-solving skills
• Ability to work independently and maintain a high level of functioning in a busy and rapidly changing environment
• Ability to evaluate and apply medical literature to direct patient care
• Flexibility as hours may be long and irregular; includes evenings, weekends and holidays as necessary.

PREFERRED QUALIFICATIONS
• Education: Doctorate of Pharmacy (PharmD).
• Work Experience:
• Five (5) years’ clinical pharmacy experience in pharmacotherapy decision-making, direct patient care and pharmacy operations OR
• Completion of ASHP accredited Post-Graduate-Year (PGY) 1 residency with two (2) years’ experience in relevant area of practice.

PHYSICAL REQUIREMENTS
• Physically agile; may require walking, standing, running, bending, stooping, kneeling, climbing, or crouching frequently.
• May occasionally include lifting or moving objects up to 50 pounds, with or without assistance.
• May require moving in different positions to accomplish tasks in various environments including tight and confined spaces, including reaching out and above shoulders, or overhead.
• May lift, turn, pull, push, or ambulate people and/or equipment with or without assistance up to 90% of the assigned shift.
• May be exposed to inside and outside environments with varied temperatures, air quality, lighting, and/or low to loud noise.
• May require working with biohazards such as blood/body fluids and airborne particles following OSHA and CDC guidelines.
• Communicating with others to exchange information.
• Visual acuity and hand-eye coordination to perform tasks.
• Workspace may vary from open to confined.
• May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
