Gifford is seeking a compassionate and experienced Lead Care Coordinator to join our Community Health Team.
In this key leadership role, you will oversee the delivery of patient-centered care across a variety of health and social service settings, ensuring individuals with complex medical and social needs receive timely, coordinated, and holistic support.
The Lead Care Coordinator provides guidance and mentorship to a multidisciplinary team of care coordinators and care managers, while collaborating closely with primary care providers, nurses, behavioral health specialists, social workers, and community partners. This position plays a vital role in improving health outcomes, enhancing care coordination, and reducing health disparities within the communities we serve.
The ideal candidate is an empathetic leader with strong organizational and communication skills, a background in health or social care, and a commitment to advancing equitable access to community-based support and services.
