What We Offer
Position: RN Case Manager
Schedule : Monday–Friday, 8:00 AM–5:00 PM
Location: Forsyth Medical Center, Winston-Salem, NC
Dept: Neuro Unit
Join our Neuro Unit as an RN Case Manager and make a meaningful impact beyond the bedside. In this full-time, day-shift role, you’ll help patients and families navigate complex healthcare needs while leading safe, coordinated transitions of care.
• Monday–Friday, 8:00 AM–5:00 PM schedule
• Leadership role in complex discharge planning and care coordination
• Collaborative Neuro care team working alongside physicians, nurses, MSWs, and other healthcare professionals
• Meaningful patient impact through individualized, proactive, and safe discharge planning
• Support for medically complex neurological patients and their unique care needs
• Collaboration with MSWs on psychosocially complex cases
• Professional growth through exposure to complex neurological conditions, post-acute care, and community resources
• Advocacy and coordination to help patients and families successfully transition to the next level of care

What We're Looking For
• Education: 2 Year / Associate Degree, required. Associates Degree in Nursing or BSN. 4 Year / Bachelors Degree, preferred.
• Experience: Two or more years healthcare experience, preferred. Case Management or related experience, preferred.
• Licensure/Certification: Current RN licensure in state of employment, required.
• Additional Skills (required): Weekend and holiday coverage, if applicable.
• Additional Skills (preferred): Knowledge/Competence of Microsoft office products and keyboarding.
