Job Summary
Under the supervision and guidance of the Case Management Director, the Care Management Navigator is responsible for supporting families through the transition from hospital to the next level of care. Working under the Director of the Case Management Department, this position helps to identify barriers to a safe and timely discharge, coordinates resources, facilitates communication with patients, families, providers, and community agencies; and supports successful transitions across the continuum of care.
The position is designed for a professional with a Community Health Worker certification who has strong knowledge of community resources, patient advocacy, care coordination, and discharge processes in an acute hospital setting. This position primarily works within the hospital.
