Required Qualifications
• Master's degree in Social Work or related social service field.
• Current Illinois LSW license required; LCSW preferred.
• Strong clinical expertise in the population served.
• Knowledge of Care Management principles and practices.
• Demonstrated leadership ability to influence practice patterns and promote effective care management.
• Strong analytical, organizational, written, verbal, and interpersonal communication skills.
• Self-directed, professional, and committed to continuous learning.

Position Summary
The Social Worker Care Manager is responsible for assessing patient needs, coordinating care across the continuum, and facilitating safe and effective transitions of care. The role collaborates closely with physicians, nurses, and interdisciplinary teams to ensure quality outcomes, appropriate resource utilization, and service excellence.
Key responsibilities include:
• Conducting psychosocial assessments and interventions.
• Coordinating discharge planning and post-acute care services.
• Connecting patients and families with community and healthcare resources.
• Addressing psychosocial barriers that impact health, wellness, and recovery.
• Promoting interdisciplinary collaboration and patient-centered care.
• Ensuring compassionate, ethical, and culturally sensitive care.
• Supporting quality, patient safety, and organizational goals.
• Upholding the mission, vision, values, policies, and procedures of Rush System.
