GENERAL DESCRIPTION
Care Coordinator reports to the department area manager and provides discharge planning services, psychosocial evaluation, counseling, and support to clients admitted to the Crisis program area. The individual works in collaboration with other department area staff to provide a full array of Behavioral Health support.
PERFORMANCE STANDARDS
• Demonstrates flexibility to adapt to changes in workload, schedule, department function and procedures.
• Participates actively in creating a positive and harmonious work environment by demonstrating teamwork and cooperative, responsive behaviors.
• Works collaboratively with the client and other behavioral health staff to formulate goals for treatment and effectively engages in discharge planning.
• Demonstrates effective communication methods and skills.
• Meets/exceeds competency criteria specific to the tasks and duties of the position.
• Demonstrates service excellence in all activities and interactions.
• Documents assessments on all evaluations
• Ability to provide back-up to other Community Crisis department positions as needed.
• Works collaboratively with clients and other staff in formulating goals for treatment.
• Clients are able to remain outside of hospital inpatient services.

MINIMUM QUALIFICATIONS
• Bachelor's degree in human services or other closely related fields or equivalent combination of education and experience.
• 2+ years working with individuals with chronic mental illness.
• Ability to work cooperatively with the multi-disciplinary team.
• Excellent written and verbal communication skills.
• Ability to verbally and physically de-escalate a client utilizing BMC trained programs.
• Ability to complete electronic charting and documentation.
• Valid Iowa Driver's License with ability to obtain and maintain Iowa Chauffer's license within one month of hire with CSC.
• For Mobile Crisis, must have valid Iowa Driver's License without restrictions.
