Citizens Memorial Hospital (CMH) is hiring a Care Coordinator to join our Emergency Department team in Bolivar, Missouri — someone who leads with skill, compassion, and a drive to positively impact every patient and family we serve.

• Hours: Mon -Fri, 10:00am-6:00pm, Flexible

What Your Day Looks Like: Care coordination advances population health by ensuring patients discharged from the ED are successfully connected to primary, specialty, and community-based services to promote the right care at the right time in the right setting. This non-clinical role plays a vital part in improving access to care, reducing avoidable emergency department utilization, and supporting positive patient outcomes through proactive care coordination.
As a Care Coordinator, you will be:
• Coordinating emergency department follow-up appointments with primary care providers, specialists, and ancillary services.
• Assisting patients in establishing primary care relationships when needed.
• Facilitating referrals and supporting continuity of care processes
• Collaborating with physicians, nurses, clinic teams, and community partners to ensure seamless care transitions
• Managing scheduling, referral tracking, and electronic work queues
• Supporting population health and patient experience initiatives

Qualifications:
Education: High School diploma or equivalent
Experience: Minimum one year of healthcare-related experience
Skills: Strong communication, customer service, organization, ability to manage multiple priorities in a fast-paces environment, and proficiency with electronic medical systems
Licensure/Certification: N/A
Physical Requirements: Ability to work at a computer 90% of the day and stand for the remaining 10%
