Expected pay for this position is $31.49 - $37.05/hour. Actual pay will be determined by experience, skills and internal equity. This is a Salaried position.
Overview
POSITION SUMMARY:
The Autism Complex Care Navigator is responsible for the development and facilitation of care plans designed to address the complex needs of individuals of all age groups as they navigate autism spectrum disorder (ASD) services, programs and supports. Provides recommendation based on individualized strength-based planning, guidance, and support needed to address barriers to care and optimize outcomes. Serves as an expert resource to the multidisciplinary healthcare team for the management of complex ASD patients including psychosocial and behavioral health needs along with complex transition planning. Utilizes motivational interviewing techniques/skills, coaching, and screening tools to assist with client self-advocacy and goal development.
Qualifications
REQUIRED QUALIFICATIONS:
Education:
Master’s degree in social work
Experience:
2 years of experience in social work, case management, care coordination or service coordination
Licensure/ Certification:
Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW)
Other Skills/ Knowledge:
Excellent interpersonal and communication skills.
Solid computer skills, including proficiency with Microsoft software.
Strong analytical and problem-solving skills, with the ability to be detail oriented.
PREFERRED QUALIFICATIONS:
Experience:
Experience working in the field of autism spectrum disorder and/or other developmental disabilities and/or intellectual disabilities
Experience working within an electronic medical record
