Job description
JOB SUMMARY
To facilitate the referral/intake process for potential patients/clients; including receiving referrals; ensuring benefits are verified; gathering necessary medical information; orders; and face to face documentation ; and coordinating care with the patient/client; their family; attending physician; and the referral source for a smooth transition into agency care.
JOB REQUIREMENTS
Minimum Education
• Associate degree in Nursing
Minimum Work Experience
• 1 year of experience as a Licensed Vocational/Practical Nurse or Registered Nurse; home health experience preferred
Required Licenses/Certifications
• Registered Nurse or Licensed Vocational/Practical Nurse
• CPR
• Current Texas Driver’s License
Required Skills; Knowledge; and Abilities
• Ability to carry out obligations and duties of position Understand the physical; psychosocial; and spiritual concerns/problems of home health patients and families
• Knowledgeable of applicable policies and procedures
• Knowledgeable of Home Health Federal/State/and Joint Commission regulations
• Nursing skills as defined as generally accepted standards of practice
• Proven written and editing skills
• Strong computer skills
• Strong interpersonal skills
